Add ADR 0044 (Status: Proposed), the single self-hosted reference
deployment composed from pinned sources, and list it in the ADR index.

Correct CLAUDE.md, which described manifest v6; the contracts accept
only manifestVersion 7 (MODULE_MANIFEST_VERSION in
packages/contracts/src/module-manifest.ts).

No code changes.
